Our case management team specializes in providing affordable and high quality senior care services to seniors in the San Francisco Bay Area since 2008 with the expansion to LA county in 2013. Our team and other healthcare professionals are here to help in a smooth transition and care plan for you. Our Certified Senior Advisors contract with many assisted living and work with placing patients in the best fit environment in alignment with where the families prefer for smooth and easy visits to their loved one. We have a personalized relationship and alliance with many of the top rated facilities in AL/Memory care to give families peace of mind in placing their loved one. Our goal is to help older adults remain independent and live comfortably in their home by helping them find the right caregiver through our referral department to assist with their needs such as: personal care, medication management, meal preparation, light housekeeping, driving to Doctor's appointment, companionship and more! We have an amazing team to provide case management once your loved one has been discharged from a skilled nursing facility, hospital or rehabiliation. We will refer you to various agencies through our referral department. Our Senior Care Advisors are knowledgeable in Dementia and Alzheimers patients and understand the needs and changes that take place during the onset. The team keeps up to date with training and workshops lead by experienced Psychologists and Therapists actively working in the senior community. In addition to our senior program assistance we collaborate with many psychotherapists providing referrals psychotherapy for our seniors and families( currently via telehealth.) Facilities are located throughout the Bay Area/South Bay and Solano County.
